Person-Centered Care

How does it work?
Person-Centered Care (PCC) is a philosophical approach to long-term care that honors, and respect the voice of residents and those working closest with them. It involves a continuing process of listening, trying new things, seeing how they work, and changing things in an effort to individualize care and de-institutionalize long-term care In Person-Centered CareImportant links:

In Person-Centered Care theindividual has the right to:
• Make decisions • Have an individual plan of care • Be included on the care planning team with the provider (such as the nursing home or home care program) • Have their hopes, dreams and goals be central to their plan • Access to organizations related to Person-Centered Care

Long-Term Care Settings

Residential Facility Class 2 (RF2's)
RF2's are residential care facilities licensed by Ohio Mental Health and Addiction Services for the purpose of providing accommodation supervision and personal care services to unrelated adult residents of facilities typically require a 24-hour level of support with daily living skills.
Assisted Living (Residential Care Facilities)
Accommodations for 17 or more adults and supervision and personal care for at least 3 of the residents and skilled nursing care to 1 or more residents. "Residential care facility" is the term facilities licensed by the Ohio Departmentof Health. "Assisted Living facility" is the more commonly used term, may be an unlicensed facility.
Nursing Facilities
Facilities that are licensed and certified by the Ohio Departmentof Health and provide personal care services, 24-hour supervision,medications, skilled nursing, and/or rehabilitation services.
Home and Community- Based Services
Services for persons who are living in the community (in their home or with a family member or friend, in a single home or an apartment building) as opposed to in a facility or institution. Such services include • Home-delivered meals• Congregate (group) meals• Adult day care senior centers transportation• Home health care (nursing, aides, therapies)• Durable medical equipment• Case management • PASSPORT• PACE and other programs
